چکیده

We report a case of a patient in whom collateral flow originating from the left anterior descending coronary artery was initially associated with a right coronary artery stenosis; seven months after successful angioplasty of the right coronary artery lesion, a severe left anterior descending stenosis developed and induced reversed collateral flow from the right coronary artery through the same collateral channel. Flow reversal in collaterals has been shown to occur in an animal model', following surgical bypass grafting and spontaneously in man'. The present report documents flow reversal in pre-existing collaterals after angioplasty. It may be a rare occurrence, but its clinical implications are significant.

Coronary wedge pressure in relation to spontaneously visible and recruitable collaterals.

Coronary angiography demonstrates only collateral arteries that are already in use (spontaneously visible collaterals). Percutaneous transluminal coronary angioplasty (PTCA) provides an opportunity to uncover collaterals ready to become functional in case of occlusion of the recipient artery (recruitable collaterals). The incidence of recruitable collaterals and their relation to the distal pre...

Potential protective effect of high coronary wedge pressure on left ventricular function after coronary occlusion.

To assess the potential of coronary collateral circulation to protect myocardium after occlusion of a coronary vessel, the mean coronary wedge pressure, the angiographic grade of collateral channels, and the left ventricular function were studied in 47 consecutive patients with mechanical recanalization of totally occluded coronary arteries. Coronary wedge pressure measurements were obtained 39...

Acute complications of elective coronary angioplasty: a review of 500 consecutive procedures.

In 500 consecutive procedures of elective coronary angioplasty attempted at a centre with a primary success of 86%, one or more major acute complications occurred in 34 cases (6.8%). Twenty four patients (4.8%) sustained an acute myocardial infarction (in six this was despite emergency coronary artery bypass surgery) and two patients (0.4%) had emergency coronary bypass without myocardial infar...

Coronary collaterals predict improved survival and allograft function in patients with coronary allograft vasculopathy.

BACKGROUND Despite improvements in the care of patients who have received cardiac transplants, coronary allograft vasculopathy (CAV) remains the most prevalent cause of late allograft failure and cardiac mortality. Few proven therapies are available for this important disease. The presence of coronary collaterals imparts a favorable prognosis in patients with native ischemic heart disease; howe...
